皮肤灌注压对肢体缺血患者创面愈合的预测作用研究进展

严重肢体缺血(critical limb ischemia,CLI)是指由客观证实的动脉闭塞性疾病导致的以一侧或双侧肢体的慢性缺血性静息痛、溃疡或坏疽为特征的状况[1]。动脉粥样硬化在闭塞性血管疾病的病理生理过程中起着重要作用[2]。CLI的每年发病率为500~1 000/1000 000,其在老年人、吸烟者和糖尿病患者中的发病率更高。亦有感染性心内膜炎导致周围动脉栓塞和急性下肢缺血的报道[3]。CLI患者今后发生心肌梗死、脑卒中和血管性死亡的风险升高,原发性截肢率达10%~40%。因其对生活质量和生存率的不良影响,CLI已成为一个重要的公共健康问题[1]。肢体缺血影响皮肤血运,这是影响创面愈合的重要因素,亦是决定是否截肢及截肢平面的重要因素[4]

对临床医生而言,预测缺血性肢体创面愈合和截肢必要性,是一项巨大挑战[5-6]。多普勒踝压力已被证明预测前足截肢愈合的能力较弱[7]。尤其在糖尿病患者用多普勒测量踝动脉压会因动脉钙化而导致结果人为升高[4]。皮肤灌注压(skin perfusion pressure,SPP)是测量肢体皮下1.5~2.0 mm微循环的一种无创技术[6],已有研究证明其在判断缺血性足溃疡愈合[8-9]方面有用。

1 SPP的测量原理

关于SPP的相关报道开始于20世纪60年代[10-11]。最早使用的放射性同位素廓清技术曾被认为是测量SPP的“金标准”,是截肢和缺血性溃疡愈合一个可靠的预测方法[12-14]。然而,因其测量方法繁琐且需要皮内注射放射性核素而无法反复测量,这个方法未能得到广泛应用。Castronuovo等[15]报道了一种新的非侵入性皮肤灌注压测量技术,测量用激光多普勒探头可以在几分钟内实现。与用放射性同位素的方法相比两者结果有很好的相关性[16]。光学体积描记法[17-18]亦曾被用来测量SPP,但在Malvezzi等[16]的研究中,该方法并非像之前报道的一样成功。

激光多普勒SPP的原理是基于测量部位的袖带产生的压力逐渐释放过程中激光对血红细胞灌注体积的测量。在受阻断的皮肤微循环恢复的时间点记录的压力水平作为该测量点的SPP值[19]

与放射性同位素廓清技术相比,激光多普勒是一种快速、简单、有效的方法,是目前测量SPP最常用的方法。除了预测创面愈合、评估缺血的严重程度[20-23]、选择截肢平面[24]外,还被用于评价动脉重建手术、血管内介入治疗和血管病变药物治疗的效果[25-27]

还有人对激光多普勒技术进行改良。Watanabe等[28]用恒温加热探头将激光多普勒探头加热至44 ℃来提高缺血肢体的SPP检出率,并认为加热后SPP升高可作为肢体缺血和皮肤储备能力的一项评价参数。

2 SPP预测肢体缺血患者创面愈合的作用

很多研究表明,SPP能有效预测缺血肢体的创面愈合,但关于其预测创面愈合的最佳截点却存在分歧。

Adera等[24]在1995年报道,在57个截肢部位进行激光多普勒SPP测量,其中大截肢36个(膝上13个、膝下23个),小截肢21个(经跖4个,截趾17个),分析表明SPP≥30 mmHg (1 mmHg=0.133 Kpa)时的阴性预测值(愈合)90%,<30 mmHg时的阳性预测值(治疗失败)75%(P<0.01)。在大截肢,阴性预测值为100%,阳性预测值为83%(P<0.01)。在小截肢,阴性预测值为75%,阳性预测值为66.7%(P<0.09)。

1997年,Castronuovo等[19]研究了61个伴难愈性足部溃疡的肢体,以SPP低于30 mmHg诊断严重肢体缺血的敏感度为85%,特异度为73%,准确率约为80%。

2008年,Yamada等[6]检查了211例动脉硬化闭塞症患者的403条下肢,其中一半患有糖尿病或正在接受透析或兼而有之。ROC曲线分析,SPP预测创面愈合的截点是40 mmHg,其敏感性为72%,特异性为88%。

Urabe等[29]研究了伴有足溃疡或坏疽的53例患者的62个肢体。患者均接受保守治疗,1个月的治疗结果被归类为“改善”(溃疡直径缩小≥20%或坏疽界限明确)和“无改变或加重”,创面的结局分为“愈合”和“未愈”(残余溃疡、加重或血管重建和/或截肢)。中位随访期为182 d (四分位距为65~584 d)。利用SPP≥40 mmHg预测1个月的结果时,其敏感度为75%(12/16),特异度为82.6%(38/46),准确性为80.6%(50/62)。ROC曲线表明40 mmHg为最佳截点。Logistic回归分析显示SPP≥40 mmHg是一个独立因素(P<0.01),比值比为14.2(95% CI:3.6~55.8)。SPP≥40 mmHg预测创面的结局时,其敏感度为61.1%(11/18),特异度为79.5%(35/44),准确性为74.2%(46/62)。这些结果与一个月时的结果相当类似,但由于17个肢体失访,它们不具可比性。

Utsunomiya等[9]在2014报道,对113例CLI患者(123个肢体)在进行球囊血管成形术前、后48 h内进行SPP检测。记录创面愈合情况,随访(17.4±12.4)个月。创面愈合率为78.9%,其SPP值明显高于创面未愈合者(44.2±15.6) mmHg vs. (27.5±10.4) mmHg,P<0.01)。Logistic回归分析表明,SPP是创面愈合的一个独立预测因子(P<0.01),SPP>30 mmHg、40 mmHg和50 mmHg时,创面愈合的概率分别为69.8%、86.3%和94.5%。ROC曲线分析表明,30 mmHg为预测创面愈合的最佳截点,具有81.4%的敏感度和69.2%的特异度。

Watanabe等[30]回顾性分析了18例CLI患者的19条下肢。动脉重建术后,6条下肢溃疡愈合,作为H组;7条下肢溃疡未愈合,作为U组;无溃疡的6条下肢作为N组。术前三组的SPP值均低于30 mmHg,组间差异无统计学意义,术后H组和N组SPP值明显改善,U组显著低于其他组。作者认为,SPP可用于评估动脉重建术后组织血液循环的改善和创面愈合的可能性,SPP值≥30 mmHg是创面愈合的必要条件。

Tsuji等[31]研究了SPP测量结果是否可以用来准确地预测CLI创面愈合和选择外周动脉重建手术, 对47例患者的69个伴足溃疡或坏疽的肢体进行回顾性分析。根据ROC曲线,SPP的最佳截点是35 mmHg。SPP对预测CLI的创面愈合是有用的。SPP≥35 mmHg是创面愈合的必要条件;SPP<35 mmHg时,在清创前行外周动脉重建是必要的。

笔者就SPP预测创面愈合的作用进行过一项荟萃分析[32],共纳入五项研究,365条肢体。结果显示,30 mmHg作为截点的敏感度为79.9%,40 mmHg作为截点的敏感度为67.1%,总的敏感度为76.1%(95%CI:为73.9%~84.9%、55.8%~76.8%和70.7-80.8%)。30 mmHg作为截点的特异性为78.2%,40 mmHg作为截点的特异性为84.2%,总的特异性为82.1%(95%CI:为61.5%~89%,74%~90.9%和73.7%~88.3%)。笔者认为,SPP能准确预测CLI患者的创面愈合。由于该项荟萃分析存在纳入研究少、样本量小等局限性,我们无法对SPP预测创面愈合的最佳截点作出绝对肯定的结论,期待更多的研究来确定黄金标准。

3 SPP与其他评估肢体缺血的方法的比较

踝肱指数(ABI)是国际上公认的评估外周循环的最常用方法。然而,糖尿病或慢性肾病患者往往伴有外周动脉严重钙化,血压袖带不能充分压缩踝动脉,因此,他们的ABI值往往处于虚高。此外,如果患者的病变位于踝以下,则无法使用ABI评估。SPP能准确评价这些患者的外周循环且不受动脉钙化的影响[24]。SPP可以评价踝以下和溃疡附近更小动脉的功能[33],预测溃疡和创面的结局[8]。Castronuovo等[19]的研究发现,ABI并不能预测是否需要血管重建或截肢,以及局部保守治疗的结果。

Yamada等[6]报道了SPP与踝部血压(ABP)、足趾血压(TBP)和经皮氧分压(TcPO2)之间具有很好的相关性(分别r=0.75, P<0.01;r=0.85, P<0.01;r=0.62, P<0.01)。Tsai等[34]的研究亦证实SPP和足趾血压之间有很强的线性相关性(r=0.87, P<0.01),无论在糖尿病患者和非糖尿病患者(r值分别为0.85和0.93,P<0.01)。并认为在足趾压力无法测量时,SPP可作为很好的取代者。

经皮氧分压(TcPO2)存在的问题包括冗长的测试时间,测量结果不稳定和受到解剖的限制。SPP是一种及时、客观、可靠的替代。一项前瞻性、单中心研究[8]通过比较100例下肢慢性创面的TcPO2和SPP测量结果来评价它们预测创面愈合潜力的准确性和有效性。设定预测创面愈合的SPP和TcPO2的基线,30 mmHg作为截点,低于该水平被视为显著异常并预示创面不太可能愈合。随访12个月或至创面愈合,以先发生者为准。这项研究的人口性别均衡分布,平均年龄为63.4岁。创面继发于糖尿病、动脉或静脉性因素。SPP预测创面结果的成功率达87%,TcPO2仅为64%(P<0.02)。此外,SPP预测创面愈合较TcPO2更敏感(90% vs.66%,P<0.01)。

Okamoto等[35]通过与多排螺旋CT扫描作比较,计算ABI、趾肱指数,TcPO2和SPP这四种无创性方法的敏感性和特异性。36例患者中有7例(19.4%)的ABI<0.9,ABI的敏感度仅为29.9%。共有41.4%患者的SPP<50 mmHg,具有84.9%的敏感度和76.9%的特异度。对于病变位于膝关节以上的患者,趾肱指数具有91.7%的敏感性和86.7%的特异性。认为SPP能预测早期外周动脉闭塞性疾病(PAOD),是检测血液透析患者PAOD的最有用的工具。

总之,SPP是一种快速简单、客观有效测量肢体血运的方法,它的优点在于测量的无创性和高度可重复性,且不受血管钙化的影响。SPP能准确预测肢体缺血患者的创面愈合。至于其诊断的最佳截点或“金标准”是30 mmHg还是40 mmHg,或者其他值,需要进一步的研究来确定。
