身体约束(physical restraint,PR)是指使用任何物理或机械性设备、材料或工具附加在或临近于患者的身体,患者不能轻易将其移除,限制患者的自由活动或使患者不能正常接近自己的身体某部位[1]。在ICU,PR常被用来预防意外拔管,确保关键操作和治疗的顺利进行。然而,越来越多的国外研究表明,PR可导致水肿、皮肤损伤、周围神经损伤[2],增加患者自行拔管[3]、谵妄[4]、创伤后应激障碍[5],延长住院时间[4],增加病死率[4]等。本文通过对2015年5月至2015年9月收治浙江大学医学院附属邵逸夫医院综合ICU的314例患者的一般资料、身体约束情况和临床治疗进行回顾性研究,以了解ICU患者身体约束的现状和相关影响因素,为ICU身体约束管理提供客观依据。
1 资料与方法 1.1 一般资料2015年5月至2015年9月收住本院综合ICU的患者。纳入标准:病例资料完整;年龄≥18岁。排除标准:收住ICU前已使用身体约束;调查结束时仍未转出ICU。使用身体约束的指征为:保证必要的治疗通路的通畅;因生理、心理等原因导致患者不能自主控制行为,有自伤或伤害他人的风险而需要采取保护性约束。患者具备上述指征之一可实施身体约束,无上述指征时解除身体约束。本研究共纳入患者314例,男201例,女113例;年龄18~98岁(62.03±15.55)岁;APACHE Ⅱ评分12~48分(21.54±7.96)分。
1.2 调查方法通过回顾国内外文献自行设计ICU患者身体约束及相关因素调查表。通过16例ICU患者的预调查,测得调查表Cronhach’ s α系数为0.85。调查表包括3部分资料:一般资料(年龄、性别、文化、抽烟喝酒史、APACHE Ⅱ评分、入ICU原发病、住ICU时间),身体约束情况(是否PR、PR时间、PR原因),临床治疗(机械通气、镇静、镇痛、谵妄、早期活动开始时间)。
1.3 统计学方法将经双人核查后收集的所有数据输入SPSS 19.0软件进行统计分析。描述性统计采用百分比、计量资料以均数±标准差(x±s)表示,计量资料单因素分析采用t检验,计数资料单因素分析采用χ2检验,以P<0.05为差异有统计学意义。以身体约束为因变量,采用Logistic回归分析筛选出ICU患者身体约束的独立相关因素。
2 结果 2.1 身体约束情况314例患者中,身体约束患者135例(42.99%)。身体约束持续时间小于1d15例(11.11%),2~3d18例(13.33%),3~7d49例(36.30%),7 d以上53例(39.26%)。身体约束原因中预防意外拔管83例(61.48%),烦躁28例(20.74%),确保治疗管路通畅15例(11.11%),预防坠床6例(4.44%),其他原因3例(2.22%)。
2.2 ICU患者身体约束相关因素的单因素分析将患者的一般资料和临床治疗资料为自变量,身体约束为因变量进行单因素分析,约束组和非约束组在年龄、饮酒、APACHE Ⅱ评分、住ICU时间、机械通气、镇静、镇痛、谵妄、早期活动开始时间差异具有统计学意义(P<0.01),见表 1。

以是否身体约束为因变量,以单因素分析中有统计学意义的因素为自变量,建立多因素Logistic回归模型,自变量均进行哑变量分析。模型采用最大似然估计法进行逐步回归分析,变量的入选标准是P<0.05,剔除标准是P>0.10,分析后得出最终模型拟合似然比检验结果为χ2=189.825,P<0.01,模型拟合良好。经多因素分析表明年龄、谵妄、机械通气为ICU患者PR的独立危险因素,镇痛和康复活动为ICU患者PR的独立保护因素,见表 2。

本科ICU患者PR率为42.99%、平均PR时间为8.5 d,高于国内朱胜春[6]的报道(PR率为39.04%、平均PR时间为20.33 h)。国外ICU的PR率为0%~100%,差异较大。南非一家医院所有ICU[7]的平均PR率为48.4%,平均PR时间为9 d,与本研究结果较为接近。本研究中较高的约束率和约束持续时间可能与护士对身体约束的作用缺乏正确的认识有关。本研究中61.48%的身体约束原因为预防意外拔管,而文献报道[8-9]60%的非计划拔管是在约束的状态下发生的,可见身体约束并不能有效预防非计划拔管。本研究结果显示谵妄、机械通气患者被身体约束的风险分别增加了3.683倍(OR=3.683)和20.662倍(OR=20.662)。Mehta等[10]及Arendina [11]的研究均认为谵妄是身体约束的独立危险因素,Mion[12]及朱胜春等[6]也认为机械通气可增加患者被身体约束的风险,可能与机械通气患者丧失语言沟通能力有关。沟通困难是ICU机械通气患者的一个常见问题,常导致患者焦虑、恐惧和愤怒,同时也可导致ICU护士挫败感和紧张,最终增加身体约束的使用[13]。此外,本研究得出大于75岁或60~75岁的患者被身体约束的风险分别是45岁以下患者的5.739倍(OR=5.739)和7.849倍(OR=7.849)。Evans [14]也同样表明老年患者被身体约束的风险是年轻患者的3倍,其中75岁以上老年患者的身体约束率最高。可能与老年患者生理功能(如视力、听力、语言能力下降[15])和认知功能改变(常伴有痴呆、谵妄[16])有关。
而研究中镇痛患者被身体约束的风险是无镇痛组患者的0.170倍(OR=0.170)。可能与ICU患者疼痛发生率高、疼痛可引发患者焦虑和谵妄有关。国外文献报道内科和外科ICU患者中重度疼痛的发生率为50%或更高[17-18]。对患者进行预先镇痛可有效促进患者舒适,减少焦虑和谵妄[11]。研究中早期活动组患者身体约束率低于不活动组,多因素分析提示2 d内以及2~4 d内开始康复活动患者身体约束的可能性分别是7 d以上开始康复活动患者的0.031倍(OR=0.031)和0.308倍(OR=0.308)。可能与早期活动可减少ICU患者谵妄、改善镇静水平和功能独立性有关[19]。Titsworth等[20]对神经科ICU患者进行早期活动后,身体约束率下降且不增加跌倒和脱管的不良事件。
