机械胸外按压:主力还是替补?

据统计,美国每年约275 000人发生院外心跳骤停(OHCA),欧洲每年约375 000人发生OHCA[1]。我国目前尚无确切的CA流行病学资料。近年虽然CA的治疗策略有所改进,但病死率仍超过90%[2]。2010年美国心脏协会(AHA)发布新的CPR指南强调高质量心肺复苏(CPR)是改善预后的关键[3, 4],但CA目击者甚至是医务人员实际操作时不能完全符合指南要求,长时间复苏过程中经常发生按压深度不足、频率较慢、按压间歇较长等问题[5]。Wik等[6]发现仅有28%的医务人员在实际操作时符合指南要求。杨正飞等[7]发现非医学专业人士在按压第1分钟虽然主观未疲劳,但已出现按压质量下降。

近年国外科研人员基于胸外按压机制设计出多种新型高效的自动胸外按压设备,如美国Thumper医疗仪器公司研发的Thumper,Weil危重症医学研究中心研发的小型胸外按压器(MMCC),韩国SungOhHwang等研发的胸骨按压配合胸廓束带同步加压装备(SST-CPR),美国ZOLL公司研发的AutoPulse束带式胸外按压器以及瑞典JOLIFE公司研发的主动加压减压式胸外按压器LUCAS等。

目前西方国家临床应用较广泛的是AutoPusle和LUCAS。大量动物实验或临床研究比较上述设备和标准人工复苏的效果,本文现对相关研究做一综述。

1 LUCAS主动加压减压式胸外按压器 1.1 LUCAS相关动物实验

Steen等[8]将100只猪室颤(VF)模型随机分为LUCAS复苏组和人工复苏组,LUCAS复苏提高心排量(CO)(0.9±0.1vs.0.5±0.1,P<0.05)、冠脉灌注压(CPP)(17±1 mmHg)vs.(10±2)mmHg(1 mmHg=0.133 kPa),P<0.05)、呼气末CO2分压(PETCO2)(2.8±0.1 kPa)vs.(2.0±0.2) kPa,P<0.05),ROSC差异具有统计学意义(93%vs.0%)93%ROSC vs. 0%)。Rubertsson和Rubertsson[9]发现LUCAS复苏时室颤猪脑血流量恢复到基础值的65%,而人工复苏仅达到40%(P=0.041),PETCO2也显著升高(P=0.009),但平均动脉压(MAP)及ROSC差异无统计学意义。Liao等[10]发现 LUCAS复苏时CPP、ROSC更高(20 mmHg vs.5 mmHg,P<0.01;8/8vs.3/8,P<0.01)。

1.2 LUCAS相关临床试验

Axelsson等[11]纳入328例OHCA患者,159例患者接受LUCAS复苏,169例患者接受标准人工复苏,两组间ROSC、入院存活率均差异无统计学意义(P>0.05)。同样研究团队在另一项观察性研究中发现LUCAS复苏时PETCO2平均值及最小值均比人工复苏高(P=0.04、0.01),间接提示LUCAS可以提高CO,但本实验未能证实两组间ROSC有统计学差异(5.9% vs.5%,P=0.69)。进一步分析发现急救人员到达现场后平均需要消耗12 min才能启动LUCAS复苏,若缩短人工复苏和自动复苏的过渡时间可能进一步益处[12],因为文献报道CA时间越长,可除颤心律越易转为非除颤心律[13]。Smekal等[14]将148例OHCA患者随机分为LUCAS组(75例)和标准人工复苏组(73例),发现两组间ROSC差异无统计学意义(30 vs.23,P=0.3),入院后4 h存活率和出院存活率均差异无统计学意义(18 vs.15,P=0.69;6 vs.7,P=0.78)。Rubertsson等[15]发表迄今样本量最大的关于LUCAS复苏效果的随机研究,纳入2 589例OHCA患者,其中1 300例患者接受LUCAS复苏,两组间ROSC、入院存活率、ROSC后4 h存活率差异无统计学意义(460 vs.446,P=0.68;366 vs.356,P=0.83;307 vs.305,P≥0.99),ICU转出存活率、出院存活率、出院后1个月及6个月存活率也均差异无统计学意义(P=0.86、0.89、0.89、0.67),本实验还比较两组同一时间点存活患者的脑功能评分(CPC),发现仅在转出ICU时两组间CPC1分患者存在统计学差异(54 vs.34,P=0.04),余时间点脑功能评分差异无统计学意义(P>0.05)。Axelsson等[16]比较同一地区引入LUCAS前后OHCA患者存活率后发现,第二阶段(2007年~2011年)即引入LUCAS复苏后,OHCA患者入院存活率较第一阶段(1998年~2003年)即标准人工复苏显著提高(31.9% vs.25.4%,P<0.01),1个月存活率也从7.1%上升至10.7%(P=0.002)。但引入LUCAS后存活率的提高可能与以下因素有关:2005年、2010年CPR指南更新;第二阶段ROSC患者更积极的治疗,如急诊PCI、亚低温脑保护、安装ICD等,实验人员的Hawthorne效应也不容忽视[17]

2 AutoPulse束带式胸外按压器 2.1 AutoPulse相关动物实验

Henry等将30只猪VF模型随机分为AutoPulse组(A-CPR)和人工复苏组(C-CPR),发现未使用肾上腺素时A-CPR可以提高CPP(21±8 mmHg vs.14±6 mmHg,P<0.0001)及心肌血流(P<0.05),使用肾上腺素后两组CPP水平均上升,但仍有统计学意义(45±11 mmHg vs.17±6 mmHg,P<0.01),同时脑血流量差异也具有统计学意义(P<0.05)[18]。Fumiaki等[19]将44只猪VF模型随机分为 A-CPR和C-CPR,根据C-CPR时胸廓前后径变化再分为C-CPR20(胸廓前后径缩小20%)和C-CPR30(胸廓前后径缩小30%),发现A-CPR提高CPP水平(16±1 mmHg vs.7±2 mmHg vs. 11±2 mmHg,P<0.05)及ROSC(14/22 vs.0/10 vs.1/12,P<0.01),心肌血流量及脑血流量也均有统计学差异(23% vs.0% vs.4%,P<0.05;40% vs.4% vs.19%,P<0.05)。

2.2 AutoPulse相关临床实验

Sergio等[20]纳入16例院内CA的患者,序贯C-CPR和A-CPR,有创血流动力学监测发现A-CPR提高患者主动脉峰压、右房压、冠脉灌注压[(153±28)mmHg vs.(115±42)mmHg,P<0.01,(129±32)mmHg vs.(83±40)mmHg,P<0.01;(20±12)mmHg vs.(15±11)mmHg,P<0.015],本实验未提供关于ROSC及存活率方面的信息,所以A-CPR血流动力学的改善是否改善患者预后尚不清楚。Michael等[21]纳入69例OHCA患者予A-CPR,93例患者予C-CPR,发现A-CPR入院存活率更高(39% vs.29%,P=0.003),进一步分析发现非除颤心律的CA患者从AutoPulse获益明显:两组间心室静止患者入院存活率比较(37% vs.22%,P=0.008),无脉电活动(38% vs.23%,P=0.079),心室静止患者获益更明显,可除颤心律患者两组间入院存活率差异无统计学意义(P=0.34)。Marcus等[22]也证实AutoPulse可以提高ROSC(34.5% vs.20.2%,OR=1.94),入院存活率(20.9% vs.11.1%,OR=1.88)、出院存活率(9.7% vs.2.9%,OR=2.27)改善明显,但两组存活患者脑功能评分差异无统计学意义(P=0.4)。同样的研究团队观察OHCA患者序贯C-CPR及A-CPR按压暂停时间(按压间歇>1.5s总和)发现,复苏前5 min,C-CPR按压暂停时间更短(85svs.104 s,P<0.05),5 min后A-CPR按压暂停时间少于C-CPR(52svs.85 s,P<0.05),A-CPR组ROSC、入院即刻、24、48及72 h存活率均优于C-CPR(OR=1.34,1.26,1.02,2.47,3.47)[23]。C-CPR按压暂停时间较长并不意外,医务人员疲劳、配合、转运等因素均直接影响人工按压暂停时间。Francois等[24]入组29例OHCA患者同样序贯C-CPR和A-CPR发现A-CPR使舒张压(DAP)从17 mmHg增加至23 mmHg(P<0.01),收缩压(SAP)从72 mmHg增加至106 mmHg(P=0.02),MAP从29 mmHg增加至36 mmHg(P=0.002),MAP增加对心脏及脑灌注压产生积极影响,虽然A-CPR改善血流动力学,但所有患者均未复苏成功,鉴于本实验采用自身对照的研究方法,无法严格说明不同复苏方式的复苏成功率。Marcus等[25]纳入1 011例OHCA患者,其中C-CPR459例,A-CPR552例,发现A-CPR组出院存活率更高(3.3% vs.1.3%,OR=1.42),A-CPR组脑功能评分为1分的存活患者也显著多于M-CPR组(12 vs.1,P=0.01)。Wik等[26]发表迄今纳入患者最多的随机临床试验比较A-CPR和M-CPR的复苏效果,前者纳入2 099例OHCA患者,后者则纳入2 132例。试验结果显示A-CPR和M-CPR入院存活率、24 h存活率和出院存活率均未见统计学差异。

3 LUCAS和AutoPulse特殊情况下的应用 3.1 院外急救转运

CA患者在转运途中的人工复苏面临多重挑战。长时间在狭小的行驶的急救车内人工复苏,医务人员身体无法固定,缺乏有效轮换,易疲劳,容易车内跌倒,严重时导致医务人员外伤[27],一项研究表明现场和转运途中C-CPR质量均下降,无血流灌注时间比接近0.5,并且50%的胸外按压深度不达标[28]。而一项旨在评价转运过程中LUCAS复苏安全性的撞击试验显示时速30 km/h的救护车发生撞击时,LUCAS与患者没有发生相对偏移,而急救人员与患者的最大偏移达40 cm[29]

3.2 急诊PCI

成人CA最常见的原因是急性冠脉综合征,目前越来越多的案例报道尚未成功复苏的患者在机械复苏辅助下行急诊PCI[30, 31]。Sunde[32]入组38例LUCAS复苏同时急诊PCI的患者,27例患者成功完成介入手术,冠脉血流均达到TIMI2或3级,16例患者PCI术中自主循环恢复,1例患者持续LUCAS自动复苏维持下转至外科手术,最终12例患者存活出院,其中11例脑功能未受损,可以想象如果没有机械按压的替代,这些患者最终都难免死亡。

3.3 急诊影像检查

部分CA患者通过询问病史、查体及床旁辅助检查不能明确病因,需要CT、MRI等影像学检查,但由于放射损伤的存在限制影像检查时C-CPR的施行。Morozumi等[33]报道3例CA患者机械复苏下完成影像检查的案例(2例AutoPulse,1例LUCAS),3例最终确诊均依赖CT检查(2例肺栓塞,1例颅内出血),值得注意的是由于CT检查要求的特殊性,3例患者均不可避免的存在按压中断时间(CT扫描为避免过多伪影,需要暂停按压),但估计中断均不超过20s。不可否认这是一项重要的尝试,但在推广这项尝试之前,需要更多的实验进一步证实风险-收益比。

3.4 移植脏器保护

在国外,生前同意捐献器官的患者正式摘除器官之前需要完成一系列伦理、法律和医疗程序,为了尽量缩短移植物缺血时间,机械按压可以保证脏器的基本灌注以维持其细胞代谢的基本需求[33, 34]

4 结语

目前,新的复苏辅助设备层出不穷,也显现出广阔的应用前景,但受限于缺乏“足够”的询证证据,对于这些新设备的临床应用仍未被积极推荐[35]

Brooks等[36]在一项Meta分析中纳入4项比较人工复苏和机械复苏的随机临床试验,发现基于有限的资料不能证明机械复苏的优势。但值得注意的是该荟萃分析纳入的试验存在明显的选择偏倚,各试验间异质性显著,没有分析比较ROSC这一更能反应单纯复苏效果的指标。因为出院出活率不仅与是否获得ROSC有关,更与ROSC后综合治疗,如亚低温脑保护、脏器替代治疗、积极的原发病治疗等有关。Mark等[37]纳入12项随机临床试验(8项LDB,4项LUCAS,总计6538例患者),发现OHCA患者LDB复苏ROSC更高(P<0.001),而LUCAS复苏ROSC差异无统计学意义(P=0.151),本项荟萃分析倾向于OHCA患者可以从机械复苏中获益。Luo等[38]纳入13项临床试验,发现机械心肺复苏可以改善OHCA患者ROSC(P=0.002),但入院及出院存活率没有差异(P>0.05)。机械复苏解决了诸多人工复苏的局限性,但到目前为止,仍没有足够的证据支持院外及院内心肺复苏时常规使用机械复苏,但在某些特殊情况下,机械复苏不失为人工复苏的一种有效替代方案,如提高CPR质量[39]、转运途中、急诊PCI等。目前仍需要更大规模、多中心的随机对照研究来进一步证实常规使用机械复苏的有效性及可行性。
